Malocclusions can have a dentogenous cause (caused by the
teeth)- these can be treated conservatively by the orthodontist-
or they can result from a malposition of or a discrepancy
in size between the maxilla and the mandible.
If the bony bases of the maxilla and mandible do not fit,
a surgical correction is needed in combination with a conservative
orthodontic treatment (brackets).
Depending on where the defect lies, the maxilla can be moved
either forward, backwards, up or down. The operation is performed
from the mouth, without any cuts from the outside, and the
bone is fixed in the new position with titanium miniplates.
The mandible can also be moved forward or backwards. This
is also done from the inside of the mouth and the bone is
fixed with titanium screws. These operations require staying
in hospital and are paid by the health insurance because they
lead to a functional improvement of the masticatory situation.
Aesthetic improvements of the patients facial profile
and frontal appearance are positive side effects, for example
the relining of a flat mid-face, the correction of a prominent
or receding chin, or the raising of the nasal tip.
The orthodontic surgery is always performed in collaboration
with an orthodontist, for the necessary pre- and postoperative
orthodontic regulation of the teeth. For the planning of the
operation we use x-rays, gypsum dies, cephalometrics, clinical
analysis and photos. We show these to the patient and discuss
the operation with him or her with the help of an anatomic
model of the cranium. If the patients wish, they may contact
our former patients for advice and reassurance.
